Report Access/Permission Change Request

What is this?

To request access or permission changes to an existing report, use this service request. 

What information will you need for this request?

The name of the report to which access or permissions should be made is a required field in this request. 

If access to a form is being requested, an Institutional Research employee will require approval from a supervisor that the individual has an educational or institutional need for this access.

The employee receiving access will also be required to acknowledge the following before access is granted: 

  • FERPA/MGDPA Acknowledgement
    • I acknowledge that the requested report(s) contains private student data that is protected under FERPA and the MGDPA. 
  • Policy Acknowledgement
    • I acknowledge I have read Bemidji State's FERPA/Privacy of Education Records policy (linked at the top) and understand the difference between Directory Information, Limited Directory Information, and Private Information.
  • Data Scope Acknowledgement
    • I acknowledge I will only access the data in the report(s) needed to fulfill my job responsibilities and that other uses of the data are not authorized.
  • Data Sharing Acknowledgement
    • I acknowledge that the report(s) are not to be shared with anyone via email or other electronic means and if shared physically the report(s) must be destroyed immediately after use. Should other faculty/staff members require access to the report(s), they must request it themselves.
  • Data Retention Acknowledgement
    • I acknowledge that the report(s) are not to be stored locally on my computer and that if local use is required that I will delete any local copies and empty them from the Recycle Bin immediately after use.
  • Data Breach Acknowledgement
    • I acknowledge that failure to comply with these precautions can result in a security incident or data breach which may bring harm to students or the institution.
